Nutritional/ Hormone Consultation
By now we’ve probably had our ten-minute phone consultation, so I have a basic idea of your concerns and what I might be able to do for you. However, there is a lot more to discuss, and that is what your basic consultation is about. Please read on to learn what you can expect, and what steps your should take before your scheduled appointment time. Don’t have an appointment yet? Your First Appointment – Basic Consultation
Your appointment will last approximately 45 minutes to 1 hour. The cost is $120.00. We accept credit cards and are open to creating a payment plan for you.
During our first visit we’ll talk about all your concerns. Often our patients come to us with highly complex or vague symptoms resulting from Chronic Illness, Peri-menopause or Menopause with need for Bio-Identical Hormone testing, Migraines, or Adrenal Fatigue – conditions that conventional health care providers often prefer not to deal with.
We are confident we can help and look forward to our first meeting.
BEFORE ATTENDING CONSULTATION:
- Please complete the symptom survey (click here) /or a hormonal questionnaire or 2 day food diary.
- The symptom survey must be filled out online
- Please provide all information as instructed on survey/questionnaire. If a question does not pertain to you, leave blank.
- Please call to schedule appointments. Phone: 201 – 869 – 5990
- Please arrive 15 minutes prior to appointment to make sure all the paperwork is in order.
- All other documentation may be delivered via US MAIL, FEDEX, UPS, IN PERSON to Atlas Drug & Nutrition 8416 Kennedy Boulevard North Bergen, NJ 07047 OR via FAX (201-869-8371).
- Please bring all current supplement containers to the appointment, if possible, and send copies of laboratory reports, saliva tests or other evaluations with your survey(s).
Other Notes About Our Consultations:
- We do not bill third party insurers but we can provide an invoice for services.
- We accept credit cards and may work out payment plans.
- Nutritional supplements, laboratory testing, saliva testing, and other evaluations maybe recommended in addition to consultation fees.
- Cancellation Policy: Please notify at least 48 hours in advance of your appointment.
- Follow-up consultation 15-20 minutes/ $60.00.
- Follow-up appointments should be scheduled immediately after consultation.
