Steps To Take Before Attending Your Hormone and Nutrition Consultation
- Record your eating habits in a 2-day food diary.
- Fill out the symptom survey online.
- All other documentation requested during the appointment process may be delivered via US MAIL, FEDEX, UPS, or IN PERSON to Atlas Drug & Nutrition 8416 Kennedy Boulevard North Bergen, NJ 07047 OR via FAX (201-869-8371).
- Provide all information as instructed on the survey/questionnaire. If a question does not pertain to you, leave it blank.
- Bring all of your current supplement containers to the appointment, if possible, and send copies of your laboratory reports, saliva tests, or other evaluations along with your survey(s).

What To Expect During Your First Hormone and Nutritional Consultation in Northern New Jersey
You can expect your appointment to last approximately 45 minutes to 1 hour. After the initial consultation, the cost of a full hormonal and nutritional consultation is $120.00. We accept credit cards, and we can also create a payment plan for you.

- We do not bill third party insurers, but we can provide an invoice for services.
- We accept credit cards and can work out payment plans.
- Nutritional supplements, laboratory testing, saliva testing, and other evaluations may be recommended in addition to consultation fees.
- Cancellation Policy: Please notify us at least 48 hours in advance of your appointment.
- A follow-up consultation is 15-20 minutes/$60.00.
- Follow-up appointments should be scheduled immediately after a consultation.
